What to prepare

  • Your name, mobile number and street address.
  • At least one supporting document, such as a valid ID or birth certificate.
  • Your bank or GCash payment receipt, the amount paid, reference number and payment date. Check the school payment instructions in the form before paying.

Choose your course and Main or SPMC branch in the application. A class group (batch) preference is optional.

Sending the form does not transfer money. The school checks your documents and payment proof separately.
